Pilgrimage to Atlanta

In consideration of Savannah Presbytery allowing the Participant(s) to participate in the upcoming group trip and its associated activities, we (I), the undersigned, do hereby release, forever discharge, and agree to hold harmless Savannah Presbytery, its directors, employees, volunteers, and agents (collectively herein the “Presbytery”) from any and all liability, claims, or demands for accidental personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participant(s) while involved in this trip and its activities.
We (I) [and on behalf of our (my) minor Participant(s), if applicable] hereby grant permission to participate fully in this group trip, including all scheduled events away from the church premises, and hereby assume all risk of accidental personal injury, sickness, death, damage, and expense as a result of participation in recreation, travel, and activities involved therein.
Transportation, Lodging, and Logistics
Further, authorization and permission are hereby given to said Presbytery to furnish necessary food, lodging, and coordinated activities for the Participant(s). Regarding travel to and from the event, it is understood and agreed that:
For individuals or families participating in coordinated Savannah Presbytery carpools, authorization is given to provide such transportation.
For individuals or families traveling independently, the undersigned assumes all responsibility and liability for their own transportation arrangements to and from the trip destination.
The undersigned further hereby agrees to hold harmless and indemnify said Presbytery for any liability sustained by said Presbytery as the result of the negligent, willful, or intentional acts of said Participant(s), including any expenses incurred.

We (I) authorize Savannah Presbytery staff, leaders, or designated adult volunteers in whose care a Participant has been entrusted, to consent to any emergency x-ray examination, anesthetic, medical, surgical, or dental diagnosis or treatment, and hospital care to be rendered to the Participant under the general or special supervision and on the advice of any physician or dentist licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital or emergency care facility.
For adult participants and families traveling together, it is understood that individuals and parents/guardians retain primary responsibility for their own medical decisions and care, but grant this authorization in the event they are incapacitated or unable to communicate in an emergency.
The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned Participant(s) pursuant to this authorization.

Should it be necessary for a Participant to return home early due to medical reasons, disciplinary action, or any other unforeseen circumstance, the undersigned shall assume all transportation costs, logistics, and responsibility for that return travel. For those traveling in Savannah Presbytery carpools, it is understood that Presbytery drivers and vehicles may not be available to provide early return transportation, and the family or individual must arrange their own alternative travel home.

The undersigned does hereby give permission for the covered Participant(s) to ride in any vehicle driven by an authorized adult chaperone or staff member while attending and participating in this trip sponsored by Savannah Presbytery. For individuals and families utilizing the coordinated Savannah Presbytery carpools, this serves as full transportation consent. For those traveling individually or as a family unit independent of the carpool, it is understood that the undersigned assumes all responsibility and liability for their own personal vehicles and transport.
